3) Fill in your information, including your name, shipping address, and zip code.
4) Print and mail your order form along with your payment, to the address provided on the form.

If you cannot print out the form, just write it out by hand, including your name, address, telephone number, email address, amount for the number of bottles requested plus the shipping and handling cost and the amount enclosed.
